Your assignment is to conduct a hazard review of the equipment involved in the “T2 Laboratories, Inc. Runaway Reaction Investigation Report” presented in class (full report available on UTC Learn). Your technical report should include the following sections:

• Introduction/Background
  • Who was T2 Laboratories and what business(es) were they involved in? Were any other parties/companies involved?
  • What was the process in question? What products were processed and what were they used for?
• Incident Analysis
  • Did T2 have a history of such incidents? Were employees aware of the hazards involved?
• Hazard Review
  • Follow the procedures outlined in class
  • Include a completed hazard review sheet (template on UTC Learn)
  • Discuss your hazard analysis: what potential deviations exist in the process? What preventative measures did T2 have in place?
• Conclusions/Recommendations
  • What preventative measures do you recommend, and how would they have prevented the incident?
  • Are any other measures needed (e.g., hazard education)?
